In consideration of my membership and my participation in gymnastics and tumbling classes, events and activities, I agree
to be bound by each of the following:
Eligibility - I agree to comply with the rules of the instructor.
Readiness to Participate - I will only participate in those classes, events and activities for which I believe I am physically and
psychologically prepared. Prior to participation, I will have practiced my exercises and will perform only those exercises,
which I have accomplished to the degree of confidence necessary to assure I can perform them by myself, and without
injury.
Medical Attention - I hereby give my consent to the instructor to provide, through a medical staff of its choice, customary
medical/athletic training attention, transportation and emergency medical services as warranted in the course of my
participation.
Waiver and Release - I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and
even death, as well as other damages and losses associated with participation in gymnastics/tumbling activities and
events.
I further agree that the instructor and owner of studio along with employees, agents, officers and directors of these
organizations shall not be liable for any losses or damages occurring as a result of my participation in the event, except
where such loss or damage is the result of the intentional or reckless conduct of one of the organizations or individuals
identified above.
For any athlete who is not yet 18 years old: As legal parent or guardian of this athlete, I hereby verify by my signature below
that I fully understand and accept each of the above conditions for permitting my child to participate in classes, events,
competitions and activities conducted by the instructor.
TUITION PAYMENT/HANDBOOK AGREEMENT
I understand that I must pay tuition to GymSouth, Inc. on the first class day of each month that my children are enrolled.
If I do not pay my tuition on the second class day of each month a late fee will be assessed and full payment of tuition and
late fee will be paid before my child can re-enter classes at GymSouth, Inc.
I understand that this will apply even if my children are absent from classes the first two weeks of the month.
The only way my tuition will be waived will be because of injury or illness that keeps my children out of classes for 1 month
or more. I must present a doctor's excuse to GymSouth, Inc. within 7 days of injury or illness for tuition to be waived.
I understand that I can cancel my enrollment at any time during the year. I must contact GymSouth, Inc. through personal
phone call or letter to cancel my enrollment. After enrollment is cancelled, I will not be liable for any further tuition.
I understand that cancellation of enrollment does not entitle me to a registration fee refund.
I understand that tuition is not pro-rated and I am responsible for full tuition payment each month my children are enrolled.
I agree to read and follow the rules and policies and procedures in the Handbook given to me by GymSouth, Inc. or online at
gymsouthinc.com.
